The glucocorticoid receptor (GR) is localized in the pituitary and in various target tissues including the hypothalamo-pituitary-thyroid axis. Few data are available concerning the GR function in hypothyroidism. In this study we have investigated the GR sensitivity to dexamethasone (Dex) in peripheral blood mononuclear cells (PBMC) in 15 patients with familial and seven patients with sporadic primary hypothyroidism. A modified Dex-induced ratio of the GR-dependent luciferase activity to that of a constitutively active reporter construct (R201A-GR) provided an index for the sensitivity of the GR. PBMC from both groups of hypothyroid patients showed a decreased GR sensitivity to Dex in contrast to a control group of patients with nontoxic multinodular goiter. The affinity of the GR to Dex in PBMC was not altered in patients with hypothyroidism. Glucocorticoid receptor sensitivity was increased by Dex both in the presence and absence of thyroid hormone in cell lines expressing exogenous thyroid hormone receptors. A decrease of GR sensitivity to Dex in PBMC from patients with primary hypothyroidism indicates that mechanisms of feedback in the target tissues might be defective during the active phase of the disease.Q:
